Confined Space Entry Permit
Atmospheric Log and Signoff
Confined Space Entry Permit helps plant and facility safety supervisors build a complete confined space
entry permit rather than a plain checklist — a time-stamped atmospheric log, a full entrant roster,
rescue provisions, and entry supervisor signoff, all locked once the permit closes.

Who it's for
Plant and facility safety supervisors who need to build a full confined space entry permit, not just a checklist.
Key features
- Space MasterRegister each confined space with its location and known hazards, so every permit is tied to the right space.
- Full Entry PermitEach permit records the entry date/time and permit time window (start and end), purpose of entry, isolation and hazard control measures in place, and acceptable entry conditions. A time-stamped log captures each atmospheric reading (O2, LEL, CO, H2S, and any other reading you configure) together with who took it and when. The roster of entrants, attendants, and the entry supervisor is recorded by name, along with the rescue provisions in place, the communication method between entrants and attendants, the means of summoning rescue services, equipment checklist items, and any additional permits required for the work (such as hot work).
- Entry Supervisor SignoffBefore entry, the entry supervisor signs the permit to approve it. The captured signature, name, and date appear on the permit.
- Cancel or Complete, Then LockWhen the job ends, the permit is closed as either Cancelled or Completed. Once closed, the permit locks for editing — any later correction is added as a separate amendment entry, and the original permit is preserved. This matches the practice of retaining a closed permit for at least a year.
- Permit PDF and Retention BadgeEach permit generates as a single PDF containing every element above. A retention badge shows the one-year-from-closure retention window as a reference, alongside CSV export of every table.
Reading items and checklist items are written in general, editable terms and are not copied from any regulation or standard. This app does not certify compliance with any occupational safety requirement — confirm your actual program obligations with your safety officer.
